S.A.G.E

 (Substance Abuse Group Education) Program

The SAGE Program is a four week educational curriculum on substance abuse. A client needs to come to the program for all four sessions.

 Thursday evening class meets from 6:00 p.m. to 8:30 p.m.

 Please discuss with the SAGE counselor if there is a need to change the time or day you attend classes. Call the office at 280-3860 extension 10, if there should be an emergency situation that would make it impossible for you to attend a class.  You will be required to make up that class as well.

 An observed urine sample is taken from the client at the beginning of the first and last class. A client whose urine sample is not “clean” will be allowed to start the program however successful completion of the program requires the urine sample taken the last class be free of all substances. You will not be successfully discharged from SAGE if the second urine test is not “clean”.

You will be required to arrive at least a half hour early for the first and last class so that all urine tests can be taken prior to class.

When attending the first class the client will sign an information/release paper this allows UCS to send your discharge certificate to the proper authority (Referral Source, Probation Officer, DHS, or parent etc.) It is the clients’ responsibility to give us accurate and complete information.

 The cost of your SAGE classes is $125.00, $65.00 of this must be paid on or before the first class session, any balance owing must be paid by your final session.
