Enrollment Requirements

The Enrollment Requirements describe the enrollment process of applying to NACS, the enrollment standards of maintaining good standing and participation with NACS, and enrollment termination.

Enrollment Checklist

The Enrollment Checklist lists all of the enrollment forms and other items that must be submitted as part of your application with NACS. This concise list is for your personal use to help you ensure that all steps are taken to complete the enrollment process. The details concerning each of these are found in the Enrollment Requirements.

Faculty/Parent Application
This form asks for basic information about the parents. Parents also serve as "faculty." Both parents must sign this application form.

Student Application
This brief form must be signed by both parents. One form for each student/child is required.

Elder/Preacher Reference Form
The purpose of this form is to indicate that the parents are active members in good standing at the local congregation of the church of Christ.
Part I of this form is to be completed by the applying family.
Part II is to be completed by an elder or the evangelist at your local congregation.

Disclaimer Form
Releasing the Mastin Lake Road church of Christ (MLR) and North Alabama Christian School (NACS), this statement places all responsibility and liability for the education of the child on the parents. A second statement also releases MLR and NACS from responsiblity or liablity for any injury or personal harm to any faculty, student, or other family member. Both parents must sign these statements.

Church School Enrollment Form
This form, required by Alabama state law, notifies your local school superintendent that your child is enrolled with NACS. The form is submitted to your local school district when all of your application materials are submitted to and approved by NACS.
You are to complete the top above Part I, and complete Parts I and III and include it with your other application materials. One form for each student/child is required.

School District Information
In order to send the Church School Enrollment Form(s) to the correct address, this form asks for your local school district's address and phone number. It also contains instructions regarding the Church School Enrollment Form. Please read these instructions carefully.

HSLDA Application

This link takes you to Home School Legal Defense Association's application page with the options to apply online, to download an HSLDA application form, or to request that an HSLDA application be mailed to you.
Membership in HSLDA is NOT required for enrollment with NACS!
Each family is responsible for their own legal defense with regard to their home school status.
Options include, but are not limited to, securing your own legal counsel, membership in HSLDA or checking with the Rutherford Institute.
