[   ]   SCHOOL DISTRICT INFORMATION - your school district’s name and address

It’s important to remember that you are not accepted as members of North Alabama Christian School until ALL of the above items are submitted and approved.

Notification of your school district via the church school enrollment form(s) is not sent until all of the above items are submitted and approved.
