This questionnaire was created for any community individual willing to offer their creative interests to youth and educate them on what they know on the topic(s) listed in the questionnaire below. Presentations would be scheduled to fit the volunteer's time during the school day. An average presentation is approximately an hour long. A short meeting is required with the Program Director before anyone can participate in the program. Please fill in the following information below.
